Case Management That Connects Needs to Support
Practical support. Intentional connection. Whole-person care.
At Intentional Steps Counseling Services, PLLC, our case management services help clients and families identify practical needs, navigate available resources, and connect with community supports that strengthen overall well-being. Services are provided by Bachelor of Social Work and first-year Master of Social Work interns under agency supervision.
Every student enrolled in our school-based therapy program automatically receives a needs assessment and appropriate case management support. Families do not need to complete a separate referral for this initial service.
Our clinical therapists may also submit an internal referral when an existing client needs additional support beyond the therapy room. Case management is individualized, strengths-based, and coordinated with the client, caregiver, clinician, and appropriate community partners.
How Case Management Supports Our Clients
-
We identify strengths, barriers, immediate concerns, and practical needs that may affect a client’s wellness, stability, school participation, or progress in therapy.
-
Support may include connections to food, housing, transportation, utilities, health care, insurance, education, employment, childcare, and other community-based resources.
-
Every student participating in our school-based therapy program receives a needs assessment and appropriate support. Our team collaborates with caregivers, clinicians, and designated school personnel while protecting the student’s privacy.
-
Intentional Steps therapists may refer current clients when unmet needs are identified. The case management team follows up, assesses priorities, and helps coordinate appropriate resources and next steps.
-
Case management complements therapy but does not replace psychotherapy, emergency services, or crisis response. Support is based on identified needs, available resources, consent, and program capacity.
